A short note about your information

Penrith Valley Regional Sports Centre uses near-miss reports to spot patterns and fix hazards before someone gets hurt.

What you share goes to the GM and our maintenance team. We may also share aggregate near-miss data with our insurer (Statewide Mutual / icare) for trend analysis — no personal identifying information leaves PVRSC unless you ask us to escalate.

Records are kept for at least 7 years. Australian Privacy Principles apply.
About this report
Notifiable workplace incidents may need to be reported to SafeWork NSW within 24 hours. We may also share information with our insurer (Statewide Mutual) or icare workers compensation where a claim may follow. See our Privacy Policy.
